This domain is 10% of CCAO-F, about 6 of the 60 questions. It pulls the earlier domains together: when a result is wrong, the cause is usually something from Domain 1, 3 or 5.

The vocabulary is ordinary. A root cause is the reason a problem happens, as opposed to the symptom you notice. Feedback is any signal about how the output did: a reviewer's comments, a number that moved, a complaint. Optimising a workflow means getting the same or better result with less time, effort or cost, for example by reusing a proven prompt instead of rewriting it each week.

The recurring test: fix the cause, not the symptom. If every draft needs the same manual correction, the prompt or the configuration is wrong, and editing each draft by hand leaves it wrong. If an answer is vague, a bigger model will not supply the context nobody gave it. The right answer finds where the problem enters (the brief, the source, the setup, the choice of feature or model) and changes that.
